Walkthrough
Step 1:
Begin the game by selecting the "Suicide Mission" difficulty. Fear not, for the game is really not that challenging. While a few sections may cause bursts of "emotion," most of the game is fairly generous. Unlocking "FUBAR" difficulty also requires you to beat "Suicide Mission," so in the interest of avoiding a third playthrough, start with that. You could easily finish the game in 5 hours on this difficulty.

Most of the achievements in the game are dedicated to the various choices you are given throughout the experience. Since you have to finish two playthroughs, just select whichever choices you want on this playthrough. During your next step, you'll focus on the opposite options.

Constantly experiement with new weapons during this playthrough. In addition to learning which ones work best for you, you'll also likely knock out most of the kill related achievements. While the sniper achievement is fairly easy (only 50 kills), some require upwards of 200, so just work at guns you'll probably never need during this step.

There are also various intel pieces scattered across the game that are tied to some achievements. You can spend time finding them all in this step or save them for the next playthrough. Whenever you pick one up, it gets saved automatically, so it doesn't matter when you decide to grab them.

Upon completeing the game the first time, if you follow this first step, you will have unlocked these achievements:
  • They Live
  • Treacherous Ground
  • The Lost Battalion
  • Desert Storm
  • The Horror
  • Three Kings
  • The Great Escape
  • A Bridge Too Far
  • Boot
  • We Were Soldiers
  • The Devil's Disciple
  • Spotter
  • Recon
  • Intel Operative
  • Marksman - Rifle
  • Marksman - Small Arms
  • Marksman - Shotgun
  • Marksman - Sniper
  • Marksman - Heavy Arms
  • Marksman - Grenade
  • Aim High
  • Applied Force
  • In Your Face
  • Blind Luck
  • Good Training
  • Situational Awareness
  • Battle Management
  • Close Combat Carnage
  • Airspace Control
  • Damn Close
  • Army of One
  • Preventive Diplomacy
  • The Human Factor
  • Deer Hunter

And also half of the decision based ones:
  • A Man of Action or
  • A Man of Patience

  • Damned if You Do or
  • Damned if You Don't

  • Friendly Fire or
  • Unfriendly Fire

  • A Line, Crossed or
  • A Line, Held

  • A Farewell To Arms or
  • Too Late The Hero

  • The Road Back or
  • The Road To Glory

Step 2
Now you're in for a tough battle. Begin the game on "FUBAR" difficulty. Be sure to make all the opposite decisions during this playthrough. While "FUBAR" isn't overly difficult, some sections borderline on cheap and the decreased health can really add to frustration in a few scenes.

In comparison to other recent games, I believe that "FUBAR" difficulty is tougher than pretty much every modern Call of Duty game, excluding "World at War." It's not as challenging as "Max Payne 3" on Hard, but is definitely much more difficult than "Crysis 2" on Delta. It also trounches most of "Dead Space."

Upon finishing this step, you will unlock the following achievement:

  • MFWIC

Along with that, you'll acquire all the opposite decision based achievements and possibly some of the weapon based ones you missed the first time.

Step 3: Clean Up
Now all that should be left are any miscellaneous achievements you missed. These will most likely include:
  • Sierra Hotel
  • All You Can Be

Since both of these require you to finish entire chapters without dying, I recommend doing them on easy (unless you are a complete monster at the game).
Story Based Achievements
They Live
Cannot be missed. You'll acquire this halfway through the first chapter.

Treacherous Ground
Cannot be missed. You'll acquire this at the end of the second chapter.

The Lost Battalion
Cannot be missed. You'll acquire this during chapter four.

Desert Storm
Cannot be missed. You'll acquire this at the end of the fourth chapter.

The Horror
Cannot be missed. You'll acquire this toward the end of the sixth chapter.

Three Kings
Cannot be missed. You'll acquire this during the eleventh chapter.

The Great Escape
Cannot be missed. You'll acquire this near the end of chapter twelve.

A Bridge Too Far
Cannot be missed. You'll acquire this during the fifthteenth chapter.

Boot
You'll get this for beating the game on "Walk on the Beach" difficulty. It stacks with harder difficulties.

We Were Soldiers
You'll get this for finishing the game on "Combat Op" difficulty. It stacks with harder difficulties.

The Devil's Disciple
You'll get this for finishing the game on "Suicide Mission" difficulty. Since you can start the game on this, you'll end up getting the previous two difficulty achievements at the same time.

MFWIC
This is for finishing the campaign on "FUBAR" difficulty. You need to first beat the game on "Suicide Mission" before you can select this. It isn't exceptionally hard, but it's certainly not a cake walk either. Here are some tips for those struggling.
  • Using squad commands can be extremely helpful. If you just can't get out of cover without immdiately taking damage, order Lugo and Adams to lay surpressing fire for you. They might even subdue the enemy with any luck.

  • Do not rush into situations. Take it slow. The game doesn't utilize unlimited respawns like the early Call of Duty games, so simply work on picking enemies off one by one. It doesn't matter if you waste a whole day doing so, the game will wait for you.

  • Don't try to conqure entire chapters at a time. If you get frustrated at one section, just try to work toward the next checkpoint, then take a break. Setting small goals will boost your own morale and help you on your way to the finish line.

  • Whenever you have a free chance, try to execute enemies. You'll be given extra ammo for your guns. Since ammo is pretty damn scarce, you'll need any extra help you can take.

  • Sometimes a piece of slab you'll be using for cover will break under heavy fire. If you find yourself dying constantly because of this very reason, try to seek out a better form of cover in the checkpoint. There is no point in wasting precious attempts simply because your cover sucks.

  • You may not always need to use cover, though. Much like the Uncharted games on PS3, you can swap the side of the screen with which your gun is aiming. This will allow you to peek around walls and land hits on enemies with them unable to return fire. It works great for any tough sections.

  • If your squadmates go down, do not rush to help them. The bleed out meter is fairly generous and enemies will obliterate you in seconds. Make sure to clear out everyone and then proceed to them. Do not send your other squad mate to heal, either, unless you know the coast is clear.

  • When given the option for stealth, use it. There is no sense in adding extra enemies and gunfire to an already tough situation.

  • Try to keep your aim as high as possible. Head shots will do wonders on making checkpoints fly by. Also make sure your reticule is red. Wasting shots will only wind up with you out of ammo and probably dead.

  • Keep an eye on the ammo for both of your guns. While you may enjoy the AA-12, if you have no shots for it, it won't do you good when you get mobbed. Make sure to always have fully loaded weapons at all times.

  • Use every type of grenade at your disposal. Stun grenades can quickly stop up to 7 enemies at a time and stickies will cause enemies to run into each other, sometimes clearing out entire groups.

  • Make sure to prioritize your targets. If a knifeman comes running, make sure he is the first target. When you see a heavy gunner coming for you, get everyone attacking him. Snipers are great fodder for Lugo, so get them out quickly.

  • Pistols are also garbage. They won't do you any favors.

Decision Based Achievements
A Man of Action
Play it loose.
At the end of chapter 7 you will have to choose whether to save Gould or the civilians. You get this achievement if you attempt to save Gould by shooting at the executioners. You must finish the level after stopping the execution in order to unlock this trophy. So be prepared to fight about 20 enemies and a Heavy. The achievement pops at the end of the chapter.

A Man of Patience
Play it smart.
At the end of chapter 7 you will have to choose whether to save Gould or the civilians. To get this achievement you must choose to save the civilians and not interrupt Gould's execution. You must finish the level after your choice unlock this achievement. All you have to do now is complete a short stealthy section. The achievement pops at the end of the chapter. I recommend this for FUBAR.

Damned if You Do
Follow your orders.
Very early on in Chapter 9 you will have to make a choice whether or not to shoot the hanging captives. To unlock this achievement simply execute one of the hanging men. The achievement will pop after you reach the next checkpoint. I recommend this for FUBAR.

Damned if You Don't
Buck the chain of command.
Very early on in Chapter 9 you will have to make a choice whether or not to shoot the hanging captives. To unlock this achievement do not shoot the captives and attack the snipers. The achievement will pop after you have killed the snipers and have reached the next checkpoint.

Friendly Fire
Show mercy.
At the beginning of chapter 11 you will be given an option to show mercy to Biggs by shooting him, thus ending his suffering. Or you can let him burn and die an agonizing death. This achievement unlocks if you show him mercy and shoot him.

Unfriendly Fire
Save a bullet.
At the beginning of chapter 11 you will be given an option to show mercy to Biggs by shooting him, thus ending his suffering. Or you can let him burn and die an agonizing death. This achievement unlocks if you walk away from him and let him suffer for his sins.

A Line, Crossed
Choose vengeance.
At the end of chapter 13 you will come to a disturbing scene. After you find out some really bad news you can choose to show restraint or succumb to vengeance. You’ll get this achievement if you go crazy and kill the citizens.

A Line, Held
Choose restraint.
At the end of chapter 13 you will come to a disturbing scene. After you find out some really bad news you can choose to show restraint or succumb to vengeance. You’ll get this achievement if you show restraint and not kill the citizens. Just fire a shot in the air or melee one of the citizens.

A Farewell To Arms
You are relieved.
This is THE decision during chapter 15. Are you crazy or not? If you really believe you'd be calling in for Dr. Frasier Crane, then have the imaginary Konrad shoot you at the end of the game. You'll unlock this achievement for your troubles.

Too Late The Hero
Carry on, soldier.
This is THE decision during chapter 15. Are you crazy or not? If you live in denial then shoot the imaginary man and plunge deeper into madness. You'll get this achievement and an Epilogue with a few more choices.

The Road Back
Live and let live.
In order to have a chance to make this decision you must choose to shoot the imaginary Konrad at the end of chapter 15. After you have killed Konrad and the credits have finished rolling the Epilogue will begin. To get this achievement, simply lay down your weapon when prompted to.

The Road To Glory
Live and let die.
In order to have a chance to make this decision you must choose to shoot the imaginary Konrad at the end of chapter 15. After you have killed Konrad and the credits have finished rolling the Epilogue will begin.

To unlock this trophy you will need to open fire on the soldiers instead of laying your down weapon. Once the firefight starts the achievement will pop. If you die the story will end but if you survive the story ends on a different note. Either way gets you the achievement
Weapon/Kill Based Achievements (Part 1)
Marksman - Rifle
Kill 350 Enemies with any rifle (Campaign Only).

The stats in this game are cumulative. This means that regardless of how many playthroughs you take, your kills will carry over. You just need to use any rifle in the game and you'll rack up kills for this achievements. The rifles include:
  • M4A1 Carbine
  • FAMAS F1
  • SCAR-H w/launcher
  • AK-47
You'll probably get this during your first playthrough, unless you stick to your weaker guns.

Marksman - Small Arms
Kill 100 enemies with any pistol or SMG (Campaign Only)
The stats in this game are cumulative. This means that regardless of how many playthroughs you take, your kills will carry over. Since this trophy also applies to SMGs, there is no need to bother shooting a pistol (though you'll have to for a few checkpoints). The guns that will get you this achievement are:
  • M9
  • .44 Magnum
  • Desert Eagle
  • UMP45
  • Micro Uzi
  • P90
Considering how meager the requirements are and how useful I found the UMP to be, you'll probably get this shortly after finding the first SMGs in the game.

Marksman - Shotgun
Kill 75 enemies with any shotgun (Campaign only)

The stats in this game are cumulative. This means that regardless of how many playthroughs you take, your kills will carry over. The requirements aren't bad, but shotguns really aren't very good. The distance is just garbage on them. It also hurts that you can't use the AA-12 towards this, as it counts as a heavy weapon. The two guns you'll be able to use are:
  • M1014
  • W1300
Just stick with a shotgun when you first find it and you'll probably bang this out fairly easy.

Marksman - Sniper
Kill 50 enemies with any sniper rifle (Campaign only).

The stats in this game are cumulative. This means that regardless of how many playthroughs you take, your kills will carry over. While 50 is a small number compared to the others, I am awful with snipers. I used them only for required moments and immediately dropped them. You will be beating the game twice, though, so you'll easily get this. The two guns are:
  • M-99
  • Scout Tactical

Marksman - Heavy Arms
Kill 150 enemies with any heavy weapon (Campaign only).

The stats in this game are cumulative. This means that regardless of how many playthroughs you take, your kills will carry over. Everyone loves RPGs and grenade luanchers, so you'll probably acquire this without even trying. The weapons you can use are:
  • FN M249E2 SAW
  • GAU-17/A
  • M134 Minigun
  • M32 MGL Grenade Launcher
  • RPG-7
  • AA-12

I don't see how you wouldn't get this, but just utilize any heavy weapon you get until it's depleted. Ammo is never really around for these and they take out so many at once that they're worth the effort.

Marksman - Grenade
Kill 50 enemies with grenades. (campaign only)

The stats in this game are cumulative. This means that regardless of how many playthroughs you take, your kills will carry over. Seeing as how useful grenades are, I don't think anyone would have trouble getting this. Grenades usually take out two people at a minimum, so just keep tossing and hoping.

Aim High
Kill 250 enemies with headshots. (campaign only)

The stats in this game are cumulative. This means that regardless of how many playthroughs you take, your kills will carry over. I found headshots to be incredibly simple in this game. The aiming is so smooth, it's almost impossible to miss headshots on easier difficulties. FUBAR will test you, though. Regardless, just repeat this 250 times and you'll have this achievement.

Blind Luck
Kill 5 enemies using blind fire. (campaign only)

I'd copy my usual ♥♥♥♥♥♥ of saying the stats are cumulative, but since this only requires a measely five kills, you'll more than likely get it during a single intense shootout or by accident. Regardless, just blindly fire at an enemy from behind cover (which means no aiming). Kill five and you'll get this.

Army of One
Kill 3 enemies with a single grenade. (campaign only)

I got this without really trying. The enemies tend to group together in this game and random grenades usually do the trick. Sticky grenades will also send an enemy running into his comrades, which should do the trick.

If you find a good checkpoint, though, you can just die and reload to try again. It shouldn't take too long. There is even a really great checkpoint near the end of chapter 14 where five enemies jump down at the same time.

Battle Management
Kill 50 enemies using only the Attack Command. (campaign only)

Spec Ops allows you to issue some very basic commands to your squad mates. If you hold the middle mouse button (by default), you'll be able to hover over an enemy and issue an attack command. Simply allow your squad to kill 50 enemies after highlighting them and you'll get this achievement.

The Human Factor
Kill an enemy by tagging him with a sticky grenade. (campaign only)

Again, I got this by accident. The sticky grenades seem to gravitate toward enemies, regardless of whether your throw indicator is even on them or not. Still, try to line up your reticlue with an enemy and toss away. You only need to tag one, so I wouldn't even bother reloading a checkpoint if you miss.

Damn Close
Kill an Edged Weapon Expert while he is up to 5 meters away. (campaign only)

This refers to the few enemies that will simply rush you with a knife. They only appear 3 or 4 times and die relatively fast. What you need to do is allow one to be practically on top of you before you shoot him. Since a headshot is a single bullet kill, just line up your sights and wait until the last second.

Preventive Diplomacy
Kill an enemy just as they are throwing a grenade. (campaign only)

I really do not have any tips for this. Enemies lob grenades at random and there doesn't seem to be any way to coax them into tossing one. When you hear an enemy claim to be prepping a grenade or see one raise their arm for a toss, just kill them quickly. You will probably do this without trying, anyway.

Airspace Control
Kill 10 enemies while they use zip lines or are rappelling. (campaign only)

Certain sections of the game will see enemies come in from the ceiling by rappel. You just need to make sure to kill them after they initiate the rappel, but before they hit the ground. You can just replay a chapter on easy if this is giving you trouble.
Weapon/Kill Based Achievements (Part 2)
Applied Force
Hit 10 enemies with your melee attack. (campaign only)

Just run up to an enemy and press Shift (by default). Repeat the process another 9 times and the achievement is yours. You cannot unlock this by performing executions, though. The melee attack needs to hit them before they are on the ground.

Situational Awareness
Stun an enemy by dumping sand on their head. (campaign only)

You actually won't get this during the first chapter when the game makes you shoot the glass window. This has to be done outside of that chapter. There are a few sections of the game where glass windows will be above enemies and I believe there is one checkpoint which can only be completed by shooting out a window. Regardless, seeing as how this is a helpful tactic, you'll probably do it without trying.

In Your Face
Kick an enemy by vaulting over a cover. (campaign only)

When sitting behind cover, you're given the option to vault it (by pressing Enter). Just make sure that when you vault a wall, there is an enemy behind it. This will initiate the dropkick and give you this achievement.

Adapt and OvercomeBlow up 10 explosive objects, killing at least one enemy each time. (campaign only)

You'll notice some red boxes with an explosive symbol on them from time to time. These can be shot and will explode. Use them to kill enemies 10 times and you'll get the achievement. One section of the game has you covering Lugo and Adams while they climb some stairs. There are at least 5 different oppotunities to get explosive kills, so you'll probably get this during a single playthrough.

Close Combat Carnage
Kill 4 enemies with a shotgun in 10 seconds or less. (campaign only)

I got this while going through the game on FUBAR. One checkpoint I was pinned down with only a shotgun and I just kept blasting away at enemies. It was tense and I thought the end was near, but I somehow managed to get 4 guys down in no time. If this is honestly giving you trouble, just drop the difficulty to easy and go on a rampage.

Deer Hunter
Kill an oryx. (campaign only)

The first opportunity you get to grab this achievement is during the first chapter. When you are prompted to rappel down a cliff, do not do so. Look over to your right and down the edge and you should see a family of these creatures.
Miscellaneous Achievements
Good Training
Sprint into cover 10 times while under fire. (campaign only)

While under fire from the enemy, just sprint for cover by holding Space (by default) and hit the wall. Do this 10 times and you'll get the achievement. You'll probably acquire this during the first chapter of the game.

All You Can Be
Complete any chapter with 60%+ accuracy without dying or reloading a checkpoint.

If you happen to be a total badass, you might be able to pull this off on "Suicide Mission" or "FUBAR." Even so, I'd recommend doing this on easy. The achievement tasks you with having an accuracy above 60% for one chapter without dying or reloading.

So, get one kill without missing a shot and then allow your squad to do the rest. Just order them to kill people and play out the chapter. I think the first chapter would be the quickest, so just load that on Easy and get to work. Try to use any semi-automatic weapons as they are single shot.

Sierra Hotel
Complete three chapters in a row without being killed or reloading a checkpoint.

Barring the chance that you are a transcendent god of gaming, this will most definitely require you to play on easy. All you need to do is beat three chapters in a row without dying or reloading a checkpoint. Since quitting the game will count as a reload, this will need to be done in one sitting.

Even with dying, I got through the first five chapters in an hour, so this shouldn't take more than 20 minutes if you skip cutscenes.

Spotter
Recover one Intel Item.

Refer to Intel Operative.

Recon
Recover 12 Intel Items.

Refer to Intel Operative.

Intel Operative
Recover all Intel Items.

I see no point in writing another guide for all of the game's intel pieces, so I'll just link you to the excellent guide already present on Steam.

Ok, so 14-15-Epilogue don´t work for Sierra hotel.
r3l1csvk 12 Feb @ 12:36pm
Also, i think i will try the Hotel Sierra if it works being done on chapters 14-15-epilogue. I mean theoretically one only have to beat chapter 14 without dying, chapter 15 is literally just a walk and epilogue, just surrender, i guess.

Thanks for the Sierra Hotel explanation. Now i know why i didn´t get it.

The "In Your Face" achievement should be ammended, saying that it is unavoidable, due to story plot.

either i missed a comment or i'm the only one who didn't get the achievement Situational Awareness (dump sand on head to stun) ... i'm not sure how many exactly but at least three windows died by my hand .. or rifle .. outside of chapter 1 and all hit 1 or more enemies. rip me?

Oh, not sure if you're still reading this, but I found out how to do "Man of Patience" in Chapter 7 and save the civilians. Just waiting doesn't do anything. You have to walk over to where Adams is, and then follow him down a ledge to ground level. Then, after you let Gould die, you follow Adams through a short sneaking bit and save the civies. Hope this helps.
