Achievement Notes.
The Charnel House Trilogy is a game made up of three chapters, individually playable at any time. There are a total of 18 Steam achievements, 12 are earned by natural progression through the game and there are 6 that are easily missed.

Chapter 1 (Inhale) = 2 missable.
Chapter 2 (Sepulchre) = 1 missable.
Chapter 3 (Exhale) = 3 missable.

If you fail to obtain one or more of the missable achievements you can as noted earlier replay any of the chapters at any time, achievements pop as the requirements are met as such there is no need to complete a chapter if just mopping up 1 or 2 achievements that have been missed.

Chapter 1 takes about 30/40 mins.
Chapter 2 takes about 30/40 mins.
Chapter 3 takes about 60/70 mins.
Achievement Guide (Inhale, Chapter 1).
Password Protected.
You logged in and tracked your parcel.
*STORY RELATED*, Alex is expecting some train tickets to be delivered by courier and they haven't arrived and she wants to check the tracking information on her computer. She has forgotten the password and only a cryptic clue on a yellow note attached to the PC tower alludes to the password. Take the yellow note over to the bookshelf and examine the book on the far right to obtain the password.
Metafiction.
You discovered Alex's identity crisis.
*MISSABLE*, when you are at the bookshelf looking for the PC login password, examine the second book in from the left and this achievement will pop.
Special Delivery.
You discovered the autobiographical element.
*MISSABLE*, after discovering that her train tickets have apparently been delivered (according to the couriers website), Alex prompts you to look around the apartment just in case they have been delivered and she did not realise. To earn this achievement you must find three hidden packages around the apartment. One is behind the radiator, one is under the lampshade on the computer desk and the last is in a plant pot by the front door.
The Breathing Method.
You suffered a great loss.
*STORY RELATED*, after you manage to open the cabinet and grab a DVD to watch, you fall asleep and are awoken by a phone call, swiftly followed by a second that leaves you struggling to breathe. Click on the bookcase and then pick up the inhaler and the achievement is yours.
The Librarian.
You saved Lang from boredom.
*STORY RELATED*, when you arrive at the train station have a chat with Dr Lang, as the train approaches, he comments that he wishes he brought some recreational reading material, when the conversation ends give him the book from your inventory and the achievement is yours.
Achievement Guide (Sepulchre, Chapter 2).
Old Gloria.
She's a stubborn old gal..
*MISSABLE*, shortly after the start of chapter 2, Dr Lang decides he fancies a drink, when you leave your cabin, examine the painting on the wall directly to the left of your cabin door (between cabin 1-A and 1-B) and the achievement is yours.
Wormfood.
You met the master..
*STORY RELATED*, on your way to get a drink from the bar you notice the door to cabin 2-C is ajar, head on inside and have a, or rather try to have a conversation with the old man within and the achievement is yours.
Good Little Puppy.
You endured Floyd's torment.
*STORY RELATED*, when you finally gain entry to the bar, speak with Floyd, then grab the flask on the counter and speak once again with Floyd. Floyd serves you a glass of whiskey for which he charges you the princely sum of five pounds, try to give Floyd your wallet and the contents of the wallet (a five pound note and a credit card) are added to your inventory, hand Floyd the five pound note to pay for your drink.

After you finish your drink a heated conversation ensues and as you go to leave, Floyd reminds you that you promised to take a drink of whiskey to Don and hands you a bottle asking for £40 in payment. Hand Floyd your credit card, the PIN can be found on your ID card, it's 1318, bottle and flask in hand, pour the whiskey into the flask and make your way back to Don. As you go to leave the bar, Floyd passes a final insult and the achievement is yours.
Pay It Forward.
You tried to save Don from boredom... but failed.
*STORY RELATED*, after giving Don his flask, he comments that nothing beats a good book along with a drink. Return to your cabin and grab the book given to you by Alex and head back to Don, he's fallen asleep, grab the key on the floor and investigate cabin 2-A. After leaving cabin 2-A, head back to Don, a black bag sits on the floor where Don once stood, examine it and the achievement is yours.
In Brightest Day.
Should've got a ring with it.
*STORY RELATED*, after discovering Don's fate head to cabin 2-C, examine the music box on the table and pick up the model train , use your ID card to pry the train apart to obtain a key. Head back to where Don once stood and use the key on the control panel and the train will speed up, return to your cabin (cabin 1-B) and this achievement is yours.
Achievement Guide (Exhale, Chapter 3).
Take The Edge Off.
You accepted Floyd's gift.
*MISSABLE*, after leaving your cabin, speak with Don then head to the bar and take/drink the Bloody Mary waiting for you on the counter.
Mean Girls.
You made Carli mad.
*STORY RELATED*, after leaving the bar head into cabin 2-C and meet Lydia, once you have finished speaking with her, head over and speak to Don about the painting of the girl (on the wall in carriage 2) and to see if Dr Lang is awake yet. When Don vanishes, head to the bar for a very bizarre conversation with... Rob.

After the conversation, grab the knife off the table and use it to cut open the painting of the girl in the 2nd carriage to get the key to cabin 2-A, unlock and enter the cabin and exhaust all conversations with your three friends within to earn the achievement.
Call Out.
You scared off your stalker... for now.
*STORY RELATED*, at some time when you leave cabin 2-A the lights will flash on and off briefly, when the lights come back on the music from within the cabin is no longer playing and the door is slightly open. After checking the cabin out and leaving once again, you hear a phone ringing in the next carriage.

Enter carriage 1 to discover that the ringing is coming from your cabin, the phone is stuck behind the seat on the right hand chair (where you started chapter 3 sitting).
We All Float.
You pulled out an old trick.
*MISSABLE*, after the phone call with the stalker, head to cabin 1-A to confront him, after your confrontation, he blocks the door preventing you from leaving. grab the inhaler from your inventory and use it on your stalker to earn this achievement.
Eyes of the Gods.
Smoking really IS hazardous.
*STORY RELATED*, with the stalker still blocking the door after your failed attempt to assault him with an asthma inhaler, take a step back and use your lighter with your packet of cigarettes to light one up. After your offer of a cigarette to the stalker is refused open your inventory to find the lit cigarette and use it on your stalker to earn this achievement.
Salvation of Judas.
You saved Carli.
*STORY RELATED*, after escaping from cabin 1-A, speak with Don outside, then head to cabin 2-A and speak with Isaiah exhausting all conversation options, once you have finished speaking with him, head next door to cabin 2-B. After the initial conversation, speak with Carli, or rather try to.

Eventually Carli will keep repeating that she "wants her silver". Speak with Sofia, she has some money you can have, it's in her bag on the chair opposite... Speak with Grub, he won't give you the bag, offer him the lighter from your inventory and he drops the bag and the money spills out on the floor. Give the money to Carli and the achievement is yours.
Lydia's Story.
You recounted Lydia's story perfectly.
*MISSABLE*, during the final scene in the bar where you meet Gavin, Rob shows up and the events of the evening of your birthday begin to be revealed. Rob offers to exchange stories, you tell him what happened to Lydia and he'll tell you what happened to Gavin.

The correct conversation options to select are...
1. "Your ex wife put her on the train to come meet you".
2. "She wasn't on the train".
3. "They never found her".
The Ex.
You found out the truth about Gavin.
*STORY RELATED*, in the final moments of the game as Rob's fate is sealed this achievement pops.

In chapter 3, can't smoke a cigarette before trying to attack Rob with a knife.
