Notes.
Reversion - The Escape has a total of 16 Steam achievements, 9 that are story related and earned as part of your natural progression and 7 which are missable which are very clearly labelled in my achievement guide below.

While there is an achievement for completing the game twice, it is possible to earn 100% achievements with just one full playthrough and one partial, you will however have to save your game at a specific spot (as noted in the Perfect Escape achievement guide section) so make sure to familiarize yourself with this in advance to avoid having to complete multiple full playthroughs to 100%.

You will also have to ensure that you do not use the in game hint system (a button with a question mark on it in the top left of the screen, to the right of the inventory button) and you will need to complete the game in under 4 hours (it should take about 30~45 minutes using this guide) to 100%.

The achievement guide below is listed in the order in which I unlocked them. At the end of the achievement guide is a full video walkthrough earning all achievements.
Introduction Achievement Guide.

Introduction.
Start a new game.

This achievement is story related.

At the start of the game, a cut scene ensues, culminating in a conversation between the protagonist (the guy you control) and a Doctor, after the conversation finishes a window opens up showing the game's controls and once this is closed the "Introduction" achievement pops.

See Video Walkthrough Part 1 from 0m00s to the 2m10s mark for further assistance.
Lady Killer Achievement Guide.

Lady Killer.
Talk to the girl.

Warning this achievement is missable.

As soon as you gain control of the protagonist and you are able to freely move around, double click the left mouse button (LMB) on the woman (Victoria) who is sitting up in bed to the right of the door, and select the speech bubble to speak with her.

Select "Hi", then "Have we met before?", then "Could you tell me anything about the attack" and finally select "I'd better not bother you" to end the conversation, at which point this achievement pops.

See Video Walkthrough Part 1 from 2m11s to the 2m39s mark for further assistance.
Sweet Dreams Achievement Guide.

Sweet Dreams.
Everyone loves a good sleep.

This achievement is story related.

After your conversation with Victoria, show her the picture in your inventory (click on the box icon in the top left or press "i" on your keyboard to bring up the inventory and then click with the LMB on the picture to pick it up, then click on Victoria to give it to her).

After the conversation, open the cabinet on the left side of the room (the thing that looks like a wardrobe with a red cross on it) by double tapping with the LMB on it and selecting the hand icon. Next double tap on the jars on the top shelf and select the microscope icon to examine the jars.

To learn what "Midazolam" is, speak with the Doctor and ask him "Doctor, do you mind telling me what "Midazolam is for?" to learn that it is a sedative. Next, grab the syringe from the table to the right of the Doctor by double clicking with the LMB on it and selecting the hand icon.

After grabbing the syringe, speak once again with Victoria and ask her to "Distract the Doctor until I say so, ok ?", and as soon as she has the attention of the Doctor, go back to the cabinet and grab the bottle of Midazolam from the top shelf.

With the sedative and syringe in hand, walk over to the guard by the door and tell him "I need to go out, it's urgent!" and respond by saying "I'm feeling sick, I need to go out now!" and the guard will now escort you to the bathroom.

Once you regain control over the protagonist in the bathroom stall, open your inventory and combine the syringe with the sedative, then use the syringe with sedative on the guard's foot exposed at the bottom of the stall door to neutralise the guard and earn this achievement.

See Video Walkthrough Part 1 from 2m40s to the 6m05s mark for further assistance.
Funny Man Achievement Guide.

Funny Man.
Read all the jokes in the graffiti.

Warning this achievement is missable, make sure to earn this achievement before you interact with the door and exit the toilet stall after incapacitating the guard in the bathroom.

To earn this achievement you must interact (double click with the LMB on and select the magnifying glass) with the four different pieces of graffiti in the bathroom stall, there are three on the left wall and just one on the right.

See Video Walkthrough Part 1 from 6m06s to the 6m36s mark for further assistance.
The Caterpillar Achievement Guide.

The Caterpillar.
Make a duct tape caterpillar.

This achievement is story related.

After unlocking the Funny Man achievement, exit the toilet stall into the bathroom and take the guard's clothes (which you will then automatically change into) and then exit the bathroom.

In the hallway, try to open the door in the top right (to the warehouse) to find that the door is locked, then go through the door in the bottom left of the hallway (which isn't locked) to enter the storeroom.

In the storeroom grab some rubber from the tyre on the floor just to the right of the doorway, then grab the roll of packing tape from the shelf on the back wall behind the ambulance sign and head back to the bathroom (bottom right door in the hallway).

Back in the bathroom, use the packing tape on the unconscious guard to turn him into a duct tape caterpillar and earn this achievement.

See Video Walkthrough Part 1 from 6m37s to the 8m33s mark for further assistance.
Big Stick Achievement Guide.

Big Stick.
Grab the baseball bat.

Warning this achievement is missable.

Immediately after earning The Caterpillar achievement pick up the stick off the ground left behind by the guard to earn this achievement.

See Video Walkthrough Part 1 from 8m34s to the 8m39s mark for further assistance.
Nuisance Guy Achievement Guide.

Nuisance Guy.
Make someone angry with you.

This achievement is story related.

After picking up the stick exit the bathroom and then exit the hallway at the bottom of the screen to the lobby and a conversation will ensue with the guard by the door, who refuses to let you out.

Once the conversation is over speak with all three guards in the room, specifically asking about the warehouse key and you will discover the guard at the door (with a shotgun on his back) has the key, but wants a password from you before he will give it to you.

On the left wall there are two notice boards, ignore the left most board and examine the board on the right with the wooden board leaning up against the wall below it and then take the Guard log after examining the notice board.

Guard log in hand, you will automatically examine it up close. Click with the LMB on the empty signature slot (to the right of the column with the single letter "R" in it) and then click on the image of the key to the right, of the right hand column.

Exit from your inventory, back to the lobby and speak with the Hefty guard on the right, leaning up against the wall near the fire extinguisher and ask "Did you by any chance forget to sign your shift log?" then after he requests that you bring it to him, open your inventory and hand the log over to him.

We now know who he is and who has the key and those names are crossed off the list.

Next, speak with the bad tempered guard on the left of the room, sitting down and repeatedly ask him "What's with you?" until he mentions that he has a headache and this achievement will pop (it should be the fourth time you ask him).

See Video Walkthrough Part 1 from 8m40s to Video Walkthrough Part 2 at the 0m33s mark for further assistance.
GateKeeper Achievement Guide.

GateKeeper.
Find the key to the storage.

This achievement is story related.

Once the Nuisance Guy achievement pops leave the poor guard alone and exit at the bottom of the screen back to the hallway. From the hallway, enter the hospital room through the door at the top left.

Speak with the Doctor once again and ask him "Would you have anything for a headache?" to learn the name of the bad tempered guard, and thus subsequently the name of the guard that you locked up and are impersonating, as well as his password, "Rojo".

End the conversation with the Doctor and exit out to the hallway and then down to the lobby and speak with the guard standing at the door. Ask the guard about the warehouse key once more, this time when asked for a password, state that it is "Rojo" and you will obtain the warehouse key and this achievement.

See Video Walkthrough Part 2 from 0m34s to Video Walkthrough Part 2 at the 2m10s mark for further assistance.

The Piper.
Hypnotize a rat.

Warning this achievement is missable.

With the warehouse key in hand, exit the lobby at the bottom of the screen to the hallway, and then use the key in your inventory to open the door to the warehouse, in the top right of the hallway.

Upon entering the warehouse, grab the matches from the shelf to the left of the door and also the newspaper from the floor in front of the wooden cupboard (piece of furniture).

Next, open the cupboard (piece of furniture) and examine it to obtain some lubricating oil, with the room ransacked of goodies look for a small mouse sitting under the "piece of furniture" in the bottom right corner of the screen.

Once the critter is located double click on it with your LMB and select the speech bubble to talk to the rat and after a little set peice conversation, this achievement will pop.

This achievement is story related.

After earning The Piper achievement, exit the warehouse to the hallway and enter the storeroom (the bottom left door) and use the lubricating oil in your inventory with the serum holder. Once the serum holder is loose, take it and exit back to the hallway.

Next use the newspaper, then the rubber and finally the matches from your inventory with the trash can in the hallway (to the right of the storeroom door) to start a fire and earn this achievement.

See Video Walkthrough Part 2 from 3m35s to Video Walkthrough Part 2 at the 4m42s mark for further assistance.
Off Down! Achievement Guide.

Off Down!.
Watch out for things falling on your head.

This achievement is story related.

After starting the fire in the trash can, exit the hallway at the bottom to the lobby and attempt to collect the fire extinguisher from the wall on the right, a conversation ensues and you eventually collect the fire extinguisher and put out the fire back in the hallway during a set piece / cut scene.

Once the fire is out, exit the hallway to the hospital room through the top left door and then look out the window of the hospital room by examining it. A cut scene will play showing a guard outside, below the window, when this ends you will remain with an external view of the hospital.

Move the cursor to the bottom of the screen and click to "look down" back at the guard on the ground floor, open your inventory and use the fire extinguisher on the guard to knock him out and earn this achievement.

Perfect Escape Achievement Guide.

Perfect Escape.
Learn How to Rappel.

This achievement is story related.

After knocking the guard out, you will automatically return to the top window with you standing at it, move the cursor over the window and then click with the LMB to return to back inside the hospital room.

Go through the door to exit back to the hallway and then use the rope in your inventory with the grating covering the window in the hallway.

SAVE YOUR GAME HERE BEFORE DOING ANYTHING ELSE.

The Perfect Escapist achievement requires you to complete the game twice, if you save here, once you complete the game the first time, you can load this save and finish the "second playthrough" by just completing the end of the game rather than having to play all the way through a second time.

Next, exit the hallway through the top left door into the hospital room and go over to the window and look out. Then use the serum holder with the rope hanging from the grating to the left, and this achievement will pop.

See Video Walkthrough Part 2 from 6m21s to Video Walkthrough Part 2 at the 7m40s mark for further assistance.
