how to unlock world map 100%? I have travelled all the passage and yet still 99%. Does ending A and B both needed for 100% Map unlock?

Kistya equip it and keep killing mobs until it breaks. Go back to NPC you got it from and ask him to repair it. Repeat a few more times until you get The Unwavering One. I added timestamps to all the figures video so it's available to see now.

My bad, there aren't 6, you start with 1 flask and have to collect 5 extra.

You are missing 1 Empty Receptacle. I have 6 in total and got the achievement giving the 6th one. Unfortunetly i can't say where i got the 6th one, it was during my normal exploration.
