This guide has answers to all puzzles, from coins to codes to doors. Useful for if you get stuck or want a puzzle reference while speed running this game. These answers are for the puzzle difficulty "normal". I am only providing straightforward codes, not locations or which doors to go through to get to what.

Eastern Vale
There is only one story puzzle within this first section. This is the Jukebox puzzle.

Items needed for puzzle:
-Button (located in the apartments southernmost of the map)
-Coin (located in Texan Cafe)
-Glue (Located at Groovy)
-Record half (Located at Groovy)

Bring all these to Neely's Bar. Take out the first record half from the jukebox and combine it with the other half using the glue. Then put the fixed record in the jukebox and put the button in place. Insert the coin in the slot and clock C2 to play the record.

You will get a key to move on.
Woodside Apartments
The main puzzle in this section is the Coin Puzzle.

Coin Locations
-The Man Coin (Located in room 206) other side is a swword
-The Woman Coin (located in the courtyard) other side is a grave
-The Snake Coin (located in the pool) other side is a flower

The code for the safe in room 206 is 13-7-11

The solution for the coin puzzle is:
Man (Empty) Woman (Empty) flower
(empty) Sword (empty) woman flower
(empty) (empty) Man Grave Snake
Last coin selection is your choice

Then you will get a key to continue
Blue Creek Apartments
The main puzzle of the Blue Creek apartments is the Clock Puzzle.

There are three clock hands you must find and place on the clock face.

Clock Hand Locations
-Hour Hand (Room 307)
-Minute Hand (Room 210-Marked with an H)
-Second Hand (Room 202-Marked with an M)

The moth room code is 373

The hour hand goes on 9
The minute hand goes on 2
The second hand goes on 3

Then you can continue
Western South Vale
The only "puzzle" in this section is the Jacks Inn safe.

The code for the safe is 0451
Brookhaven Hospital
When you first enter the hospital,go into the secretary rooms. Inside one of them will be a padlock.

The padlock code is 724.
This will allow you access to the basement, where you can get the elevator running.

Now you need to find three bracelets.
-Marked Bracelet (Located in medical records)
-Bloodstained bracelet (located in the pool)
-Filthy Bracelet (Located in D1)

The code for the D1 room is 4-37-12

Nurses Lounge code is 3578

When you get all three bracelets, go to the Directors office.
Here will be a hand you put the bracelets on.

Top bracelet is Bloodstained bracelet with 792 facing out
Middle is Marked bracelet with 45 facing out
Bottom bracelet is the Filthy bracelet with 71 facing out

The hand will drop a key. This key is used to unlock a door in the same room. In this room you will find a button. Go back out by the hand and open a curtain to your side. Insert the button in this spot.

The code for this puzzle is is in this image.

Then you will get a rooftop key to continue
Brookhaven Hospital Otherworld
The chained box is the main puzzle in this section.

Take the elevator to the second floor where you will find the chained box.

Keypad code is 9659 and the lock code is 1622

The key is located in Exam Room 4.

When you open the box, a cutscene will play. Afterwards you will get a elevator key. Go use the bottom in the elevator and descend to the designated floor.

There will be a riddle in the elevator. The answers are 2,3,1. You can put them into a box in the Pharmacy room for some extra goodies.

Now you need to find two rings.

The first ring, Copper ring, is on a table in the pump room.

The second ring, Lead ring, in a refrigerator in the Doctors Lounge.

Now bring these rings to the room with the chained box, and put them on the door with the lady. It does not matter which ring you place first. Now you are able to continue.
Eastern South Vale
There are no puzzles in this section, besides answering a phone call and getting a wrench to open a gate.
Historical Society
To get the key to open the Historical Society, you have to look in a small box in front of a statue at Rosewater Park after a cutscene with Angela.

Continue through the building, and you will eventually enter a room full of roaches that will lock behind you. There will be a key in the center of this room. The code for the door is randomized but it will include the following digits (in no order) 2,3,9. Keep trying combinations until you unlock the door.

Continue until you reach a gate that will require the key you just retrieved.

Now you can continue into the next section.
Toluca Prison
This prison puzzle is a bit complex. You need to get weights but in a specific order. The weight puzzle is located in the yard.

Where Weights are and what door they are behind
-1st is in the chapel when you first enter the prison
-2nd is already at the main puzzle location in the yard
-3rd is behind the headless serpent door (electric puzzle code: 645132)
-4th is behind the hornless ox door (in 2F showers)
-5th is behind the eyeless boar door (Warden's Office 3F)
-Last weight is behind the wingless dove door (in the basement)

Door Unlocks
-Headless Serpent: Left Side 1 Right Side 6
-Hornless Ox: Left Side 1,6 Right Side 4
-Eyeless Boar: Left Side 1 Right Side 4,5,6
-Wingless Dove: Left Side 1,3,5,6 Right Side 4
-Sword (Not a door but the final puzzle stage): Right Side 2.3.5 Right Side 1,4,6

The next Part of this puzzle is the riddles. It is different depending on how you match everything but here is how I did it.

Poem I- "I watched them burn..."
Poem II- "What were those reasons..."
Poem III- "My only daughter..."
Poem IV- "So my guilt is plain..."
Poem V- "You were oh, so..."
Poem VI- "In truth he was..."

Then pull the noose labeled "VI"

Then you will drop into a corridor where you can continue.
The Labyrinth
The Labyrinth consists of a few phases for this puzzle.

Fist you will have to defeat Abstract Daddy, which is the most simple part of this section.

Once you kill him, you will make your way to the main part of the puzzle. Go visit Maria first to open the Rotating Cube.

Rotten Area

The first area you will enter is the Rotten area. Rotate the cube to an upright triangle with a cross. (left, left, down)

Just a heads up, this section is full of Mandarin monsters. Dodge when you hear them.

For this part, you will need to lower a cage using a lighter you will build. You need three items, lighter gas, lighter wick and a broken lighter. To get to the broken lighter, you will need a key which you will find in this area. Once you have found them all, combine them and go to where the cage is and light it on fire and it will drop. Here you can continue to the next area.


Desolate Area

This is the second area you must complete. Rotate the cube to an infinity symbol with two lines above it. (left, left, down, down, down)

For this area, you need to go up and down specific ladders. You will need to find a key to unlock a gate that is handcuffed. Go down ladder II, up ladder I, down ladder III, get the key, go back up ladder III and then you can unlock the gate.

Then make your way to ladder IV and climb down it.

Next you will need to cut through a fence using wire cutters. Go forward until you see a set of two big double doors. Break the wall and go up ladder V. Continue until you find the cutters then return to the gate. Just a heads up, Pyramid Head roams this area freely and will attack you. Once you cut the fence you will be able to continue to the next area.


The Ruined Area

Rotate the cube to a upside down triangle with a longer cross. (left, left, down, down)

The ruined area does not have puzzles within, just a series of trials essentially where you fight monsters or stay alive until the alarms stop.

Once you finish these trials, you can take the other half of Pyramid Head's weapon and go through the wall full of monsters. Then you can continue to the next section of the game.

Lakeview Hotel
When you get to the hotel, you will encounter a music stand. You need to find three figurines to place inside the stand.

Snow White Figure

There is a mirror puzzle you must complete to get this figurine. The mirror puzzle is in room 202. You need to put the mirror pieces together like a jigsaw puzzle. One of the pieces is in the bedroom across from the mirror wall. Place an apple on the table after putting the mirror together. Then go into room 201 to collect the figurine.


Cinderella Figure

For this figure, you need to access the third floor. Go into the Conference room on the third floor. There will be a key there. Bring the key to its designated area and from that room, go into the garden and you will come across a Fireplace key on top of a yellow mop bucket.. Bring that back to the conference room, where you can then get the Cinderella figurine.


Little Mermaid Figure

This figure is longer to get. First, go into the Reading room. Collect the four books laying around and put them in this order:
Top Left: Revelations Top Right: The One Who Soared
Bottom Left: Stalwart to the End Bottom Right: Pride Before The Fall

Then enter the room and the suitcase codes are DEED DONE. You will then get a elevator key for the employee section. Go down the elevator after putting all of your belongings in the cabinet. Go straight ahead and in a small squared in area, you will find a safe. The code is 7414.

Then go to the basement door and go through that area. It is by the squared in area. It has a big exit sign right above it. You will find a bar key. The last figurine is in the bar. Make your way back to where your belongings are then go to the music stand.

Music Stand Solutions

Snow white goes on the bottom left. Little Mermaid at the top and Cinderella on the right. Then rotate the circles until they go through the lines.

Then rotate the first key 7 times, the second 5 times and the third once. Now go to room 312.
