Here's how it works:
If you just started the game, stop everything and do the vehicle side missions first. You’ll thank yourself later. As the story goes on, gangs around the city will become hostile and start shooting at you on sight. This makes missions like Paramedic, Vigilante, and Firefighter much harder if you leave them for later. It's way better to complete them early while the city is still calm and safe to drive around.

PS: To keep track of your progress throughout the game, go to https://gtasnp.com/upload and upload your save file. There, you’ll get detailed information about your run, including what you’ve completed and what’s still missing.

Paramedic
To complete it, you need to reach level 12 in a single run. You cannot fail, exit the ambulance, or accidentally hit the patients. They tend to run straight at your bumper when you stop, so be careful. Slow down a few steps away and let them run to the back of the vehicle. The best place to do this mission is in Portland at the very beginning of the game. The hospital is close to everything, and the streets are small and quiet. Once completed, this mission gives you infinite sprint, which makes the rest of the game much easier.
Firefighter
You need to put out 20 fires in Portland, 20 in Staunton Island, and 20 in Shoreside Vale. The game keeps track of your progress, so you can take breaks. Just grab a Fire Truck from any station and start the mission. Fires will spawn in parked cars. Spray them from a safe distance and avoid crashing. It’s a good idea to finish this mission early in Portland, before the gangs become aggressive. Shoreside Vale has tight turns and hills, so it might take more time there.
Vigilante
You need to kill 60 criminals in total, 20 in each island. You can use a Police Car, Enforcer, FBI Cruiser, or even the Rhino tank later if you unlock it. Like Firefighter, your progress saves between sessions. The easiest method is to trap the criminal’s car and wait for them to get out. Most of the time they won’t fight back, especially early in the game. If they’re on foot, just run them over or shoot them quickly. Criminals carry weapons from Shotguns to Flamethrowers.
Taxi Driver
This one is straightforward. You just need to complete 100 fares using a Taxi or a Cabbie. You can finish it whenever you like. Passengers appear with a blue arrow, and all you need to do is drive them to their destinations. The mission is pretty relaxed compared to the others. You can even earn bonus tips for fast driving if you reach the destination quickly.
