Gnomish Mines (ARMOR Sphinx’s Casque)
Secret area hidden behind lever puzzle. Reward: Sphinx’s Casque – a legendary helmet with +3 armor, +8 Intelligence (serviceable quality, repairable to +4 AC).

Minetown 50% chance to appear
Several merchants and contains 3 chests, a Strength Potion, and 2 Crystal Shards.

Portal to Underworld (WEAPON Khryselakatos)
Use portal to return later. Reward: Khryselakatos – chance to fire off any special ammo type in the game.

Jungle Temple (Green Orb)
Green Orb in hidden chest with 4 Strength Potions (left path before floor exit, collect gold on the way). Used later for trades (see table below).

Haunted Castle (ARMOR Oracle's Treads)
Oracle's Treads – artifact boots with +3 armor and Lesser Warning (reveals nearby moving entities). Found on the second floor of a tower with 4 Levitation Potions. A lever in one tower opens 3 hidden chests behind a waterfall.

After the Baron or Baphomet. In the sewer system of Hamlet, there is a unique shopkeeper that takes your orbs.

Sokoban (ARMOR Djinni’s Braces)
Push all 17 boulders into pits to earn the Djinni’s Braces – legendary gauntlets with +3 armor and +1 Magic Resistance. Serviceable (+4 AC when repaired), high gold reward in hallway. Avoid breaking more than one boulder for artifact to spawn.

Minotaur Maze (Red Orb&WEAPON Gungnir)
Reward: Red Orb and Gungnir – a divine spear. Red Orb used later to trade (see table below). There are a few Speed potions

Mystic Library (Blue Orb)
Secret area containing the Blue Orb and several spell books. Two hidden chests: one requires levitation or map-breaking; the other requires a wall to be broken. This floor Contains magic-themed loot. Blue Orb used later to trade (see table below).

Underworld you can retreat only on the first floor
This level features Baphomet as the boss instead of the Baron.

If Accursed is in the party, the Mystic Library will contain the Spellbook of Vampiric Aura.

Defeat the cockatrices to claim the Dragon’s Mail – artifact heavy armor with +5 armor and +1 Health Regeneration. Durable and life-saving on late levels.

Bram's Castle (ARMOR Wraith's Gown)
Secret room grants the Wraith's Gown – artifact cloak offering permanent levitation and +1 Magic Regeneration.

I forgot to say you also forgot about the secret in Minetown that has 3 chests, a strength potion, and 2 crystal shards in it.

Minetown is only a 50% chance to spawn and if you change your mind on the first floor of the second Underworld you don't have to fight Baphomet.
