puzzle 5 was a little more tougher than puzzle 4 though. the platform to the exit was a little sadistic. that was great! xD
puzzle 99 was fairly straight forward, although i would of liked to of been in those other chambers solving puzzles there at the end of that puzzle. :P
I hope you`ll do part 4 soon.

It was made in Hammer. You can get Hammer by running the Portal 2 Authoring Tools in the Steam tools section. The Valve developer wiki is a good resource:
