For the new court position decision, the AI check interval is still 60 (5 years). An updated version of this mod would reduce this down to 6 like before, but this interval check would also apply to the other possible court positions as well.
Therefore, this should work fine, is still needed, and does not need an update.

Updated for v1.8.*. No changes.
When I first published the mod, to allow the AI to take the decision to get a doctor at all, the associated event had better options for the AI that were more or less the same as the players. I never found it performance intensive.
It's definitely fixable if someone wants to mod that event. Note if you use all my mods I also need to include health_events.txt in my Random Date Black Death mod so beware your edits will clash with that.

@THE DEVIL'S ON THE LOOSE
Nothing's changed since the v1.5 Royal Court/Fleur-de-lis update. Vanilla AI will take the decision and hire doctors, but will only check the decision every 5 years, and not if they are at war. The mod makes it so the AI check every 6 months, even if they are at war.
Updated for Fate of Iberia v1.6. No changes.
Updated for Royal Court/Fleur-de-lis. Tweaks the hire court physician to increase AI uptake of doctors in line with v1.5 enabling AI to take the decision; AI will check the decision every 6 months rather then every 5 years, and can take the decision even if they are at war; they will now need 30 gold. Adds/keeps player alert when the doctor position is vacant.
AI will only check the decision once every five years vs twice per year in this mod.
You can't be at war to hire a doctor.
You have to have 30 gold.
You can only take the decision once per year, versus once every three months (more a limit on the player).
I think having the AI only check every five years is still a big problem. I'll update the mod to bring in the vanilla tweaks but have the AI check much more frequently.
It could be done, but that part of the code is handled in a vanilla event that this mod doesn't touch. This mod tweaks a vanilla decision to allow the AI to also take the decision (and to make it a major decision so you get an alert), the way the decision works in the mod and vanilla is to trigger a vanilla event that does the grunt work of hiring the physician and paying him. TBH I don't think the physician cost is worth creating a modded game rule over, it's paired to generic minor (30) and major(?) (100) gold values for balance depending on the quality of the physician, I don't see how much would change by tweaking the costs.
Would it be possible add a rule in the game setting that A.I get a discount on the physician cost? like 25% 50% 75% or Free ?
