Kinda false advertisement, don't you think?
I just tested it myself and ran into a problem when disabling a hud mod then enabling this mod before starting a game. The game seems to hold onto data from deactivated mods.
I'm not sure if it's a bug with L4D2's scripting engine (or perhaps it caches mod data) but restarting the game after disabling hudmod / enabling D&D fixes the issue.

-Head to:
SteamApps\common\Left 4 Dead 2\bin
,right click vpk.exe send to desktop.
-Drag each mod onto vpk shortcut.
-Copy "materials" folder from D&D into hud mod's folder.
-Open hud mod's script folder, open hudlayout.res with notepad.
-Insert following code in between overview & hudcommentary:
Dust&Dirt
{
"ControlName" "ImagePanel"
"fieldName" "dust&dirt"
"xpos" "c-512"
"ypos" "c-256"
"zpos" "c-1000"
"wide" "1024"
"tall" "512"
"pinCorner" "0"
"visible" "1"
"enabled" "1"
"image" "hud/dustanddirt"
"scaleImage" "1"
}
Save, then drag edited mod folder onto vpk shortcut. Done!
Although I think all that it would change is the weapon selection menu by placing it back to default position.
